At level 3, Paladins gain divine health which says they are immune to disease due to “divine magic flowing through you”. Occasionally “disease” is mentioned with some spells and monsters, but I haven’t seen a clear definition of what is considered a “disease” in this system. I am a pathologist by profession and I’m sure my definition of disease is more broad than that of 5e. Am I just missing a definition or sidebar somewhere?
A paladin is immune to all disease. That includes spells like Harm and Contagion, which are diseases. Usually these diseases are caused by a spell, monster ability or just exposure, and you are immune to all of them.
As stated in the above answers, Divine Health renders you immune to any disease contracted in the game as a result of, say, being bitten by something, coming into contact with a sick person, or being affected by a spell that causes disease like Contagion. Being subjected to a saving throw to avoid a disease is not even needed; your deity protects and purifies your body.
As a side note I'd like to think that this makes a Paladin immune to certain effects of aging like arthritis, cataracts, or osteoporosis! All the more to keep them in good fighting form later in life.
Diseases will include all the ones contained in the Dungeon Master Guide Disease entry, as well as any effects from Traits, Spells, Attacks or features that specifically inflict a disease.
