Well its any cookie that gets too large...so it is a valid question for each person that is impacted, which cookie is the culprit. But getting to the right one isn't exactly something I want to walk my mother through.
If we could get a list that might help the developers. On the other hand, I have no idea where this issue is on their priority list. I'm sure they can mine the server logs to know how often it happens.
Circling back to assuming it's extensions (such as Beyond20 or AboveVTT) causing this issue, I present my evidence that you need to stop assuming things and actually look into this.
I'm at work now and do not have any dndbeyond related extension loaded in my browser (Chrome):

All those blank spots are supposed to be PC avatars, right? I can go ahead and do the work around I posted about here then CTRL+Shift+R to Hard Reload the My Campaigns page and now get this:
So unless Chrome extensions on my home laptop are causing issues with dndbeyond's site cookie on a completely different computer, this issue is not caused by or related to a browser extension.
This is still an issue and has not been addressed by staff in any way that actually solves the problem in a meaningful way. Can we get an update on this by staff? (I mean, probably not, but I'm willing to keep poking...)

Another forum regular asked if I had seen a survey recently about the upvote system. I must have missed that myself but if it is indeed true, is a positive sign.

I'm visiting the forums today. When I'm done, I'll have to clear my cache as well before I can depend on any image hosted by dndbeyond.com to not return an error and load correctly. I could go get a screengrab right now and I'd bet you a shiny nickel there's a bunch of images that failed to load on my character page or my campaigns page.
I permanently blocked the ratings cookie a while back and I went from having to clear that one cookie multiple times a day followed by a hard refresh to get images back to never having to deal with it. You'd think they would have come up with a solution by now (or at least addressed it more head on) if all it takes is blocking 1 cookie.

I'd be interested in how to permanently block that portion of the site cookie.
While on dndbeyond click the EditThisCookie icon (looks like a cookie)
Scroll down until you find a line that reads: .dndbeyond.com | Ratings
Expand that line and click the "block" button as circled below. The first two boxes should be checked - leave the value box unchecked. Then click add this rule.
Do a hard reload and possibly empty cache: In chrome hit F12 then right click the reload button and choose hard reload You may have to choose empty cache and hard reload if the rest fails.
Some people have also mentioned they've had to log out and back in after following these steps.
With this a few of us have been able to fix this issue and stop it from reoccuring.
Haha, not the first time (and not the last) that I see Jay coming up with a little solution to fix a long going client-side issue. :D
I do confirm that the Rating cookie is basically a way to store, client-side, the items you upvoted, instead of having to generate requests to the server everytime (so not much of an issue if you don't have TONS of stuff upvoted - still it's an ever growing cookie...). However, I find that it's a bit lazy on the dev's side - it should be cached server side, not client side to lighten up the code. I'm guessing the check for the cookie is probably done in JS, which would slow down browser anyway so... kinda shovelling snow forward. :D
That being said, I would guess that the forum themselves are an heavily modified version of another commercial software and not something build from the ground up (who does that in 2022 anyway?).
Does the extension "cookie editor" have a block function? I'm trying to avoid installing yet another browser extension for this.
(Would I be better off installing "edit this cookie" and deactivating "cookie editor"?

That's excellent to hear, that it doesn't remove functionality!
I haven't yet run into the image-loading issue myself, so I'm not going to use the cookie blocker, and I'm not sure we need to recommend people use it in general unless they have this issue. Just on the off-chance that the cookie is serving a useful purpose in reducing server load for normal site traffic by less-prolific users, which is gonna be most of the traffic.

I've been having issues with images for months (since sometime last fall I think. Definitely since strixhaven). Not all images, but most.
I'm on mobile chrome. Never had any extensions. Clearing site data did not fix it.

I use the extention EditThisCookie it has a block option. You probably don't need this but for anyone that wants a walk through here's a video.
Edit: I also want to note I had this issue long before I used AboveVTT - before I even knew it existed.
