I Will be posting in the order of your posts and then I will update the map. So if at first things seem chaotic, it will all work out.
Xavier moves to about 15 feet northeast of the pool. From his position he can see that the choker is about 15 feet up the tree in a sort of nest. He can see another choker in the southern tree guarding the entrance (C1), which is also 15 feet off the ground.
Eiren makes his first move and the cast Chill Touch at the Choker, but misses. Looks like you didn't post your second turn, please do so and roll initiative. From his position Eiren can see two more chokers in the southern tree guarding the entrance (C1 & C2).
Baik moves to about 15 feet east of the pool and casts his Holy Flame at C3. C3 Dex save 4, the choker is unable to dodge the flames and is slightly cooked. From his position Baik can also see C1.
Corvin trys to move into position to attack the choker, but when he passes through the space between the two trees (Q21), C1 makes a readied action. First Tentacle Attack: 24 Damage: 7 bludgeoning + 3 Piercing damage. The choker's tentacle grabs Corvin lifting him into the tree 5 feet below the choker (10 feet above the ground). Corvin is grappled (escape DC 15). Until this grapple ends, he is restrained. Corvin make a Str save DC 15. On a failed save, you are also paralyzed for 1 minute. (You can make a new save at the end of each of your turns).
About the same time as Corvin is attacked, Velmine is as attacked, just before she tries to hide. The second choker in the southern trees attacks Velmine with it's first Tentacle Attack: 10 Damage: 5 bludgeoning + 6 Piercing damage. Followed by it's second Tentacle Attack: 20 Damage: 4 bludgeoning + 2 Piercing damage. The first attack misses, but the second Tentacle grabs Velmine lifting her into the tree 5 feet below the choker (10 feet above the ground). Velmine is grappled (escape DC 15). Until this grapple ends, she is restrained. Velmine make a Str save DC 15. On a failed save, you are also paralyzed for 1 minute. (You can make a new save at the end of each of your turns).
The bear comes charging in and C3 using its readied attack to attack the bear. first Tentacle Attack: 21 Damage: 7 bludgeoning + 5 Piercing damage. Killing the bear. Well it was a good bear.
Xavier's frostbite zippes through the trees at C3. C3 Con Save 13. C3 is feeling a little cold. (I'm assuming that you dropped the Sickening Radiance, if you didn't I won't be included it in the map, unless there is a need to do so in the future.)
Eiren launches two arrows at C1. The first hits while the second misses (see the next post).
Corvin and Velmine make your Strength Saves, Baik and Corvin roll initiatives, if you are over 20 you can act, assuming Corvin is able.

(Just for the sake of asking, I don't have any reactions this turn, do I?)
I don't think you can use Uncanny Dodge because you didn't see the choker. However I don't see a problem with Hellish Rebuke. I forgot to mention that the tentacles that are restraining you both are wrapped around your necks. So you would be able to point a finger, and the paralyzes doesn't start until the end of your turn, which was interrupted by the readied attack.
(Great. Hellish Rebuke was exactly what I was going for, I just needed to know when the paralysis started. It could've been just after I failed the roll, in which case, not even Hellish Rebuke would be an option. I don't think a riposte on the first attack would be possible either, since the enemy wasn't within my melee range at that point, or was it? Anyways, here we go.)
Velmine is completely taken by surprise since she assumed the first choker to be the only one, but as the tentacle hits her neck, she points upwards casting Hellish Rebuke, which is a blast of fire that hits within the top of the tree, and close to the choker who attacked her.
Damage:17Fire (Dex Save 16)
(It can't burn the tree, by the way... All the spells that can put fire on flammable objects are stated as so in their descriptions. That is not the case for Hellish Rebuke, so that's my assumption.)
As she is being pulled up into the tree, Velmine is cool and collected and manages to cast her defence spell. The choker is engulfed in flames, killing it. The tentacle is still wrapped around your neck (I'm going to say it takes a bonus action to unwrap it at this point), but with the choker not resisting, Velmine falls to the ground (no falling damage due to the fact that you weren't high enough yet). Pulling the choker out of the tree atop her. Unfortunately Velmine is still in the tree and is paralyzed (unless you can come up with something I'm missing, before your turn ends).
If you can't think of anything go ahead and roll your second Strength save since you are ahead of the chokers in initiative. (Even though you are paralyzed, for this purpose only your Strength saves do not automatically fail).
(In theory you could use riposte as the choker is attacking you with his 10 foot reach natural weapon. The tentacle would have to be in your melee to attack you. In practice I probably wouldn't apply the damage if you hit, but you would have foiled the attack.)

C1 tightens his stranglehold on Corvin Tentacle Attack: 21 Damage: 5 bludgeoning + 5 piercing damage, and then he does it again with his second Tentacle Attack: 7 Damage: 9 bludgeoning + 5 piercing damage. The first attack hits while the second misses. C1 then uses his Aberrant Quickness to repet his 1st Tentacle Attack: 21 Damage: 9 bludgeoning + 7 piercing damage, leaving his second Tentacle free to attack someone else. Tighten his stranglehold on Corvin even further. (Corvin is not paralyzed so the attacks are not ccriticals I don't have time now to look at the damage dice now. For a total of 13 Damage.)
C3 moves down his tree and over to Velmine and attacks her paralyzed body with his first Tentacle Attack: 10 Damage: 8 bludgeoning + 5 piercing damage, missing her, so he will try again with his second Tentacle Attack: 22 Damage: 9 bludgeoning + 8 piercing damage, wrapping the tentacle around Velmine's neck. Not that it really matters, but Velmine is grappled (escape DC 15). Until this grapple ends, she is restrained. The 3rd choker then moves into to tree moving up 5 feet.
Xavier sees movement as another Choker (C5) moves out of the shadows 15 feet east of his position, stopping 10 feet from Xavier's position and attacks Xavier with his first Tentacle Attack: 10 Damage: 7 bludgeoning + 5 piercing damage, followed by his second Tentacle Attack: 7 Damage: 6 bludgeoning + 2 piercing damage, both miss. The hunger choker uses his Aberrant Quickness to try again with his first Tentacle Attack: 9 Damage: 6 bludgeoning + 5 piercing damage, followed by his second Tentacle Attack: 13 Damage: 5 bludgeoning + 2 piercing damage. Xavier is feeling pretty good as all four attacks miss.
But then he sees a second choker emerge from the shadows (C6) and also try to attack him also. The new choker's first Tentacle lashes out Attack: 25 Damage: 8 bludgeoning + 6 piercing damage. This time her tentacle wraps around Xavier's neck. Xavier is grappled (escape DC 15). Until this grapple ends, he is restrained, on top of that Xavier can’t breathe or speak until the grapple ends. Because he can't speak neitherEirenor Baik doesn't knows that there are two more chokers to the east. However Baik can make a free perception check to see if he sees them, even though his attention is focused south DC 16

A creature can hold its breath for a number of minutes equal to 1 + its Constitution modifier (minimum of 30 seconds). Since you didn't know you would need to hold your breath, I don't think that this part counts. Any thoughts?
When a creature runs out of breath or is choking, it can survive for a number of rounds equal to its Constitution modifier (minimum of 1 round). At the start of its next turn, it drops to 0 hit points and is dying, and it can't regain hit points or be stabilized until it can breathe again.
For example, a creature with a Constitution of 14 can hold its breath for 3 minutes. If it starts suffocating, it has 2 rounds to reach air before it drops to 0 hit points.

A creature grappled by the monster can use its action to try to escape. To do so, it must succeed on a Strength (Athletics) or Dexterity (Acrobatics) check against the escape DC in the monster's stat block. If no escape DC is given, assume the DC is 10 + the monster's Strength (Athletics) modifier.
So my Str is +0 my dex is +3 and my Athletics is +4 so I’m going to roll a straight d20 and DM can let me know if i can use my athletics or if it just has to be a straight strength check to break

Eiren takes two quick shots at the disgusting creature holding Velmine (If that one is already dead, he'll go for the one high up in the tree, C1) while Qu'oth flies down to help him.

Corvin is able to free himself for the choking tentacle, maybe because Eiren's two arrows kill the beast as he is trying to free himself. He is about to cast Spiritual Weapon when he realizes that all the choker's are dead. (Take back the Spiritual Weapon), instead he pulls Velmine out from the trees
Velmine you can roll 9 more Str checks, or just wait out the minute

Corvin is able to free himself for the choking tentacle, maybe because Eiren's two arrows kill the beast as he is trying to free himself. He is about to cast Spiritual Weapon when he realizes that all the choker's are dead. (Take back the Spiritual Weapon), instead he pulls Velmine out from the trees
