The winged hybrid swoops down with incredible speed. His mind is focused and he won't be fooled by Mumed's trickery this time. Successfully bypassing the cleric's Sanctuary, the monk lays four well-placed hits on the human. Mumed does not have enough time to react (Mumed takes 46 magical bludgeoning damage).
Blood covers Mumed's entire set of robes. He is not looking very healthy.

Keeping within melee range from his assailant, Xavier circles around Graxx and stops at {CC22}. From his new location, he says: "Why don't you go for a stroll?" as he hits a couple of strings in his musical instrument.
Yessaris eyes are ever moving as scenario after scenario plays in mind. And again, he found himself coming to the same conclusion as time; stacked odds, or not. Eyeing the axe in hand with some distaste, he looks to Graxx and the others. "Make the most of this second try. There won't be a third." He calls out to the others just after giving his axe a shake. Projecting from its tip is a beam of light that soon wraps up Graxx in a veil of yellow.
"Remember the device, dear Graxx. Do recall the gift I bestowed that little crossbow of yours. You got eight shots, so make the most of them for our other friends, won't you?"Without going into further explanantion, Yessaris takes what may be a risky step back just to turn his axe head on Mumed. From the top of its handle comes a new light - a bead of fire - that tries to find purchase upon Mumed.
-> Attack is supposed to be at advantage because of Adamantine Half-plate. Rolling an additional die here: 14.
Yessaris is grabbed by a floating piece of fabric, and then shocked briefly by the gnome (Yessaris takes 17 lightning damage and cannot take reactions until the beginning of his next turn). Now farther away, Yessaris throws a specialized shot at Mumed. As before, the cleric does not find an opening to react, and is burned on the chest (Mumed takes 12 fire damage but is still up).
-> Over-sized Greataxe vs. Graxx: Attack: 32Damage: 19 magical slashing (see edit below).
(@Grx: Because you mentioned it in you most recent post but was unable to use it, I'll go ahead and assume you want to use your reaction to half the damage of the most damaging strike above through Uncanny Dodge. Let me know if that is not the case.)
Edit: Here are the Sanctuary rolls for the three attacks above {DC 18}: 12, 6, 7 -> only the second attack goes through.
Edit: The attacks above are not supposed to have advantage because of Graxx's Elusive feature. Taking the first roll for each, the first and second attack rolls remain unchanged, but the third one becomes a 18 instead.
In the end, Baern swings his axe only once. Graxx manages to mostly avoid it, making the hit nothing more than a glancing blow (Graxx takes a total of 12 magical slashing damage).

Edit: Graxx gets a lot more than what he bargained for. Thankfully, he manages to slip sideways so that no vitals are hit (Uncanny Dodge - reference; Graxx takes a total of 41 magical slashing damage).

Battered, Mumed focuses on magically closing his own wounds first. He audibly calls on Ilmater to Preserve Life, healing himself for 70 HP. Not satisfied, the human cleric then throws in an upcast Mass Healing Word for good measure.

It would have worked even better than before, but the artificer’s armor protects him from the worst of it. Nonetheless, the yuan-ti is temporarily held in place as the lightning travels through his body (Yessaris takes 20 lightning damage and cannot take reactions until the beginning of his next turn).
The illusionist's Dragon then lands, its upper torso, neck, and head now poking through the southern wall. As before, it releases its breath straight down, from no more than 15 or so feet up. The area of effect is much smaller than it could have been, but it still encircles Graxx and Baern. The blast seems to purposefully avoid Xavier, while the dwarf doesn't seem surprised by the friendly fire.

The acid rain also does some damage to the mangled metal body that used to be called Attorney. The creature has been disabled for some time though, so this additional damage means little...
Still dealing with the gash left on his torso from Baern's devastating attack, Graxx reacts a bit too late (Graxx takes 27 acid damage). Baern does not make the same mistake, avoiding most of the splash (Baern takes 13 acid damage).
(@Grx: This damage should be enough to force Graxxunconscious, incapacitated, and prone, but you just regained your reaction at the beginning of your turn... hint hint.)

Selrahc focuses but manages to only fully extend his arms once towards Mumed (only the third attack goes through Sanctuary). Because Tupoc made Selrahc's aim a bit more true (i.e. Bless), the maul avoids Mumed's shield. Just before it meets the human's face however, the cleric's whip snaps against the hybrid's wrist, changing its path enough to force the swing to miss.
With Abe on the floor, Milo turns to his other target and moves to {BU18}. From there, he shoots his crossbow at Yessaris once again:
