Probably the best game from my group would be when an orc (fighter) a kobold (sorcerer) a harpy (ranger) and a gargoyle (bard) were exploring a long forgotten temple of Yume, the god of stonemaking and deep dwarves.
They came across a room with four pedestals, each with a handprint on it. When they all put their hands on them they heard a large booming voice say "what is your greatest weakness?"
Trying to remove their hands didnt work so they all answered and made a con save, the Kobald and the orc failed and were given a lv of exhaustion, making 2 for the orc.
They then placed their hands back on and were asked "when were you most humbled?" again they answered and made a dex save, the orc and the harpie gaining a lv of exaustion
They then placed their hands on again and were asked "When did somebody sacrifice something for you?" they all answered except for the kobald who was in a cold sweat (her mother had died trying to save her from a dragonborn lord)
The groups hands had been taken off the pedestals and were trying to ask what was wrong which only made her struggle more, trying to yank her hand off it.
The bard tried using detect thoughts to try and find out what was wrong but she had a ring of mind shielding on so it didn't work.
The fighter tried to get the spark (the questgiver) to possess her so that it could speak for her, but she passed the save
She then looked the fighter dead in the eye and said "you call youself my friend, cut off my hand"
He then tried to break the pedestal with his warhammer, breaking the warhammer
now she was trying to gnaw her own hand off
The fighter swings at her arm with his hand axe
The bard parries it with his quaterstaff
He swings again

The fighter then tries to push the bard over but the bard makes the save
The fighter then uses the bards own bardic inspiration to beat it
The Kobold now only has one hand
They then tried to go to sleep but the temple was enchanted so they could not.
The bard then has the idea to use the tiny hut spell to get away from these effects
But guess who wasn't allowed in
When they woke up he was at four levels of exhaustion, later dying to the boss of the temple
Moral of the story ladies and gentlemen, never EVER piss of a bard.

It was a traumatic experience she hadn't properly dealt with yet
