I mean, that's pretty reasonable. A trained ninja can't just stand in the middle of an open field and not be spotted. Hiding in 5e is more about not being heard than not being seen, they kinda clarified it in the UA, but still not great. Though if you want to play as a ninja you should take the Skulker feat that allows you to hide when lightly obscured. A big beefy barbarian shouldn't get ninja-level stealthiness.
I'm pretty sure you in your life have snuck up on people with far less concealment than heavy. It is not reasonable in the slightest. It defies reality. It makes your fantasy characters worse at stealth than the actual player.
No, I've never been able to sneak up on someone who is looking in my direction without heavy concealment. You'd need to use the .. what's it called... the optional rules for directional looking and vision cones? Then everything outside of a creature's vision cone is heavily obscured to them. I'd agree with having those rules graduated up into base rules rather than optional ones (the climbing on giant monsters should be made a base rule too), because I use them already for whenever players are stealthing outside of combat.
Sure actually returning facing to the game would help with the issue, but you should not need it. The system uses abstract rules for its combat and environment and that is what the skills should represent. If I successfully rolled my stealth roll to hide, and beat your perception in that open field that means I am approaching you form a direction outside of your vision cone and not making enough noise to draw your attention to me. Just explain the logic to the players/DM in the rules, it is abstract as there is no facing which way a character is looking is abstracted, the skill represents things such as approaching from the direction they are not looking. Add some general statement that in some situations a DM might decide stealth is impossible, such as trying to sneak past a door that someone is guarding as that is no longer a situation where their facing is abstract it is concrete, they aren't just standing around they are just directly looking a the door with no obstructions. And a DM may impose disadvantage an or give advantage to the target in situations where its not impossible but difficult.
As an aside I suspect you have got the drop on people even while they are facing you in less than heavy concealment. A busy environment, especially when people are distracted or lost in their thoughts, is not heavy concealment but its pretty easy not to see someone in a city etc until they are right upon you. Maybe you stand out in a crowd so you have not experienced this, but I have. Heck in busy enough environments (Disneyland) I have seen it happen even when the person was actively looking for them.

I know there used to be a whole section in the PHB about it, and now it is only briefly mentioned in the glossary. I know it used to be that Hiding was against a creature's passive perception and now it is against a DC 15.
I also know that skill check contests rules are completely gone from the PHB, and grappling was a skill check contest and is now a save against a fixed DC. Has anyone heard what they are doing for Deception vs Insight skill check contests?

God they really just murdered Ranger, why did they think giving 1d10, with concentration, for a few times a day, at level 20 would be a good idea? Literally 2014 guidance is better because it’s a cantrip you can spam, they didn’t even think to remove concentration? Of course they removed most of the exploration flavour and abilities.
