There are so many downsides it seems like that it was not thought through at all.. The effect on bonus actions is absurd. It's a functional two weapon fighting nerf. It's a functional nerf to any paladin spell with "smite" in the name. It makes the "help" action super confusing. It makes healing somewhere between 20% and 33% worse, since it seems like death saves would be made with a single 1d6 initiative.
But more than the end effects, it's just not how melee fighting works. There's never a time in a fight when you're not at least passively thinking about moving. That's how attacks are made. You consider your spacing and anticipate your opponent's movement to look for an opportunity.
And the upside is that certain actions are typically quicker than others, making combat more realistic? That's what speed factor does. Just use that.
