The genesis of appetite is a small concave hand mirror.
It functions like normal mirror except is makes character flaws perceivable.
When flaws are perceived the mirror morphs into a mask unbeknownst to the user.
(User sees the same mirror regardless if they're told about its apparent change in shape).
The mirror offers to make the user flawless if they look close (meaning they move closer to the mirror).
If the user complies they roll of the following table.
(edit: the same effect from a mirror feature may not be gained multiple times, it still creates a reflection and the user loses a level,
but may not reroll on the table.)
The mirror alters the very fate and soul of the person using it.
The price for using this item is simple, the user loses a level.
The level is absorbed into the mirror and a perfect reflection of the user is made somewhere else
(edit; multiple reflections may exist at once, this includes if the same user uses the mirror multiple times or if a different person uses
the item. All limits on stats and level remain.)
This reflection is convinced that it is the true person the user embodies and will kill them on sight,
because its the best possible version of the user.
If the reflection dies, it is regenerated within 1d10 days at the place it died.
If the user dies the reflection also dies and the user's levels are absorbed into the mirror.
The levels absorbed this way sustain the powers of the mirror.
The reflection has the user's stats but each stat increased by 4 and the max stat limit increases to 30.
A Reflection also gains experience as the user does. Reflections have a challenge rating of 0 for experience purposes.

With the following changes:

Both the reflections and monsters created by the mirror gain the benefits of any effect the mirror granted to the user.
(edit: They gain the benefits even if the user uses it again after they were created. EX. a user uses it once and gains option 01-02,
reflection also gains the benefit. The same user uses the mirror again, another reflection is created and both of them gain the benefits
of the 2 effects.)
01-02: Take +4 in one stat and -2 to another, neither one may increase/decrease the same stat. Can be gained multiple times.
03-04: Increase effective caster level by 1, if not a caster pick a casting class, you count as level 1 for spells and spell slots
05-06: Gain the ability (Action): once per long rest; gain the benefits of a short rest and 1 level of exhaustion.
07-08: Gain a level 1 class feature from any class.
09-10: A +1 weapon appears on the floor next to the user at the next dawn.
11-12: A potion of supreme healing appears in the users pocket, it refills every dawn.
13-14: The user becomes infected with vampirism (Consult DM guide and MM)
15-16: Summon an Aspect of death under the user's control, it cannot regain hit points.
17-18: A mark appears on the hand of the user. The next time the user kills a creature they may gain its stat replacing their current stats.
19-20: Gain ability (Triggered) : once per long rest, the user may forgo filling a failed death save.
21-22: Reduce Constitution to 10, you may move all point lost this way to other stats including the one Constitution. Can be gained multiple times.
23-24: Gain proficiency in one skill. can be taken multiple times.
25-26: Take -2 to a stat, gain any feat you meet the requirements for.
27-28: The user may take another subclass along with their current one.
29-30: For the next 3d6 day, A sack of gold containing 300 GP appears next to you every dawn. Can be gained multiple times.
31-32: Gain 1 legendary action with the following uses; make one attack at a target in range, move up to your movement, Dodge.
33-34: Gain Proficiency in one saving throw including death saves.
35-36: Gain feature exquisite resting. When the user takes a long rest they regain all hitdice in addition to its normal effects.
37-38: One stat of the becomes an 18. Can be gained multiple times.
39-40: The user gains enough experience to increase by to levels at the next dawn. Can be gained multiple times.
41-42: Gain the ability (Action): once per week you may use Time stop taking the max result and will not stop until the duration is up.
43-44: You gain advantage with 1 saving throw, the user gains resistance to the damage caused by the saving throws effect.
45-46: The user becomes infected with lycanthropy, user choices what animal.
47-48: The next time the user would die, they are resurrected and lose 1 level. Can be gained multiple times.
49-50: Gain ability (Triggered); Whenever the user would regain hit points, regain double the amount
51-52: Gain ability (Triggered); Whenever damage is taken reduce it by the user's proficiency bonus.
53-54: The user no longer needs to sleep and instead may perform light activities.
55-56: The user gets a playing card, the playing card acts like a scroll of Heal expect it may be used by non-spellcasters. Can be gained multiple times.
57-58: Gain ability (Passive); Users hit points are increased by 50.
59-60: Gain ability (Reaction); If the user is attack or must make a saving throw, they may spend a reaction to gain +2 to AC and saving throw respectively.
61-62: The next dawn a ring of protection appears on the user's finger.
63-64: The user creates an additional reflection and gains +2 to all stats. Can be gained multiple times.
65-66: Gain ability (Passive); all damage to reflections is doubled.
67-68: Gain ability (Triggered); damage dealt by the user heals the user for half the amount inflicted.
69-70: Gain ability (Passive); The user gains one conditions immunity mentioned in the MM.
71-72: The user loses an extra level and gains +2 to one stat. Can be gained multiple times.
73-74: The next dawn the user finds a chest with a note addressed to them. Inside is a potion of speed that refills at the next dawn.
75-76: Gain resistance to damage from reflections.
77-78: The highest level reflection appears next to the user If the reflection is killed the user gains the reflection regeneration ability.
79-80: The user loses levels equal to the number that user has created, all current reflection are destroyed and don't regenerate. Can be gained multiple times.
81-82: Gain ability (Bonus action); The user may as a bonus action teleport 10 feet in any direction.
83-84: The maximum number of attuning slots the user have increases to 5.
85-86: The user becomes immune to exhaustion.
87-88: The mirror has no effect. Can be gained multiple times.
89-90: The mirror places a brand on the user, The user is vulnerability to damage done by reflection. The user may scry all reflections with no limitation.
91-92: The user gains access to a demi-plane which they have complete control over.
93-94: Gain ability(Action); the user may cast Heal on a creature that isn't themselves once per long rest.
95-96: A piece of parchment is added to the user's inventory, The parchment will cause anyone to show to be charmed by the user for the next 24 hours.
97-98: every time the user hears the Phase "I wish..." the user has Geas is cast on them with the instructions to care out the persons wish, should the user succeed the user may have a wish granted.
99-00: Roll 2 additional times on the table. Can be gained multiple times.
