Don't forget that both Mezzoloths are still under the effect of Bane. Therefore, Mezzoloth 3 will miss Khessa since 19 - 1d4 < Khessa's AC 19.
Since Avery wears Adamantine Splint, critical hits count as normal hits. Therefore, even though that first attack hits, it should just do normal damage, not critical damage.
Of course, that Mezzoloth probably didn't land a critical hit since it should have been attacking with disadvantage because Avery is still protected by Theldrin's Protection from Evil and Good.
You said, "against the widening Cloudkill." If that's just the regular Cloudkill, I don't think that the AoE grows (RAW). The spell description just says, "the Sphere moves 10 feet away from you at the start of each of your turns." That is, the AoE is still just a 20-ft radius sphere, but the that sphere moves 10-ft away from the caster, presumably moving west into S35 in this case.
If Mezzoloth 1 was the one that cast the Cloudkill, it needs to make a CON saving throw for concentration.

Xanrym enters the next room, which looks to be a closet. Inside are a number of outfits that look to fit Fadheela as well as a large stone chest and a staff propped up against it. When he searches the north end of the room, he finds another secret door.
Round 7: Xanrym
Xanrym turns to call over his shoulder to Theldrinand Syllen, "It's a sort of closet with Maroe's robes, a wizard's staff, and a chest. But there's also a hidden door at the back." Looking back, Xanrym sees the yellow-green fog -- thick as ever -- just as Coriana emerges from it. He hopes that Khessaand Averyare still alive and fighting the Mezzoloths with the support of the berserkers, but he doesn't know how he can help them with that poisonous cloud blocking their way.
Either not realizing that the poisonous cloud is moving (and about to open a clear path to get into S32) or seeing that the cloud is moving (and worried that the poisonous cloud will soon cover the north end of the room too!), Xanrym announces, "I'm going to see where the secret door leads!" He tries to open the secret door, hoping that it gives them a way to escape the poisonous cloud and somehow rejoin the fight against Orinielle. (The other secret doors in the Necromancer's Spike were easy to push open once Theldrin found them.)
Xanrym's action will depend on whether he can open the secret door and what he finds when he does:
If there's a monster behind the door, he'll probably attack with a spell.
If there's a long tunnel that continues beyond the edge of the light or that turns east or west, he moves forward and may simply dash, hoping to find that it leads them back to S32 somehow.
If there's another room or a shorter tunnel, he'll move forward more carefully and Search the space for threats or other secret or hidden doors.
Movement: Up to 30-ft of movement, through the secret door, before deciding his action. Action: Cast a spell (if he encounters another monster) / Dash if it's a long tunnel, or Search the next area (Perception: 17) Free Action: Talking, Opening the secret door
Once within the next 1 hour, when the you fail a D20 Test, you can use the bardic inspiration to roll 1d8 and add the number rolled to the d20, possibly turning failure into a success. If you use the bardic inspiration, but the D20 Test still fails, you keep the inspiration die because of Xanrym's Unfailing Inspiration.
(OOC: @Aaggie - it looks like the Cloudkill had already moved past Avery this round, so he doesn't need to save and won't take damage. Also, remember that Avery has a 1d8 Bardic Inspiration die from Xanrym in case you want to roll it to boost that attempt to break the grapple. Because of Xanrym's Unfailing Inspiration, there's no downside if the roll still fails to break the grapple.)
Syllen, annoyed that he can not get close enough to heal the main fighters or get line of sight teleports next to the Deva. Syllen gives a quick prayer to the Cosmos that he will not get attacked by the Deva.
