if it hits 4 damage and Then make a dc 11 save or 11 poison damage.
Make that a critical, so add 4 damage...
Guessing Grahl is going down... Still make your con save, since the poison damage might not be included at least. These bugs sure roll good against Grahl for some reason.

Orgen stands in front of Grahl looking suspiciously. "Oy, Lupin can you get Grahl back up," he asks. He holds is hammer out and ready to smash anything that looks like it could attack.
Until now they had not found anything that hinted at a supernatural presence, but the skirmishes they fought against the local fauna already began to take an increasing toll on their bodies. Alek started to get some doubts if they were prepared enough to handle the situation. But he also knew that the time for second thoughts had been long gone. If they wanted to establish themselves as a reputable mercenary company, the only way open to them was to proceed with the mission. "Somebody help Grahl to get back on his feet. If we can sweep the floor successfully we might have some time to rest an recuperate.", Alek told his companions.
Grahl is awake and at 1 HP. You have just verified there are no more threats in this kitchen area. You have seen, before the combat started, a set of tracks coming from the door to the north and going to the door to the east.
What do you do now? Do you continue looking through the house, do you head back to town for a long rest, do you take a long rest here, do you follow the tracks, or do you do something else?
"Friends, do we have a way to patch Grahl up quickly, so we can sweep the floor and try to get a rest afterwards?", Alek considered the state of his companions for a just a moment before continuing, "Or we could retreat for now, set up camp at a safe distance for this house and then come back well rested. Or do you guys have any other suggestions?"
(OOC: I can only heal myself, you do have lay on hands do you could give him 5 hit points to patch him up. Also, I don't think he used his hit dice from the last short rest so he could potentially heal more.)
Orgen points out the tracks to the group and says, "I think I see where are hunter friend went to. My guess is that the vampire is in that direction. We could try to pursue them, but Grahl should probably stay in the middle of the group if we take that course."
The immediate danger now past them, the kobold takes a moment to swipe the centipede guts from her blade and to sheath it, before dismounting quickly to walk over to where Grahl is coming around. She carefully places her tiny hand on him, urging some of her own vitality to transfer over and heal some of his injuries. It isn't much, but she gives him an encouraging smile all the same, turning to the rest of the group to join the discussion after she removes her hand.
"Dezlo has no objections to either pressing on or to going to rest. We were not expecting the house to have such impressive pests; and Dezlo is not remembering us having a strict timeline to finish investigating the house. It is not terribly hard to get back here, so we can always return to our tower and come back rested and better prepared, yes?"
OOC: Dezlo expends all of her Lay on Hands usages to give Grahl back 5 HP.
"Should we make a vote of it then?" Belle asks with a tentative smile at the group. "Isn't that how these franchise things do these sorts of things..." She looks at the group with a innocent look that melts your hearts. "I personally vote to continue on if Grahl is up to it."

Ok then, Grahl, give me a death saving throw...
Belle will try to bandage the wound, though she is bad at medicine...
Medicine: 14
She puts on a fairly sloppy covering over the wound and applies pressure, stabalizing Grahl. No death save needed... He is at 1 HP.
