Shade, when throwing you can only attack once, not twice and you only have a +3 to attacks, not a +4 because your dexterity is 13, not 14. Also, your first attack misses short of the tentacle and clings to the ground, off of where it moved on the wall, it seems to be faster than your throw. It then reels back and whips at you Shade. 19 is the attack and 10 is the damage.
(Thought darts were light which would allow double-throwing (they are finesse which would allow throwing in off-hand). Also, sorry about the modifier. I had the stats arrranged for a while with 14 dex and forgot to adjust the modifiers when I changed to 13. Also, Medic!)

No, the finesse does not allow throwing in offhand, just usage of strength or dexterity to attack and deal damage with. This means that you can use darts while using strength instead of the dexterity, not throw two separate darts. Forget that post below, the two-weapon fighting rules state that you cannot make two attacks with two weapons that are not light. So if they were daggers this would work because you can still do this with light thrown weapons. To throw both darts at once, you would need the Dual Wielder Feat.

The Heavenly Swordsman rushes forward, seeing that ranged attacks seem to fair poorly, before jabbing at the shadows with his daggers [Attack roll 19]. His first swing strikes its mark [Damage roll 12], and his second dagger is sent to follow his first [Attack roll 11].
Just saw I forgot to add a + sign between the 4 and 2, so ignore that roll (I'm not deleting it because that might trigger a manipulated rolls banner). Use this one instead: [Damage roll 4]
And I assume that the second misses, since Barlow missed with a higher roll.
Your dagger seems to slice through the thick shadowy creature and ping the wall in the process as you have noticed every other successful attack has done the same. The wall takes the damage.

HS then gets violently pulled forward by the shadow and is pulled into the clutched of the shadow taking 1 necrotic damage and if this is more than two, then you are not only knocked down but consumed by the shadow and it regains however much damage it dealt as health. This means you die.
Edit: HS's body is consumed in the dark manifests of the shadow and his gear disappear with him as the shadow seems to enjoy its "meal".
