I’m entering into an epic campaign (going to lvl 20 and beyond), with every possibility that the DM will encourage us to make gestalt characters, since there’s only three of us. I’m hoping this will be the “final” world for my world-walker barbarian chieftain, with his legacy weapon and the commission of the goddess to find a power strong enough to break the fate of his home world. My problem is that I’ve used most of “the usual” multiclass options for a half-Orc barbarian (i.e. Battlemaster, Champion, Moon Druid, and Rogue). I want something fresh and powerful, something that is really only feasible in a gestalt campaign to epic levels, but I’m torn between going with what I know works (the usual) and what sounds interesting and flavorful (i.e. Conquest Paladin, Hexblade Warlock, or Grave/Forge Cleric). I may be overthinking it, but I’d love to get y’all’s opinions and advice/recommendations. TIA!
I would definitely go with something that has magic. Probably cleric or warlock. Then again, you could play something totally out of this world (ie) An artificer. The artificer has magic and the same type of magic as a bard, has the same health as the better of the magic users, and you get the tinkering. The tinkering would be amazing for a heavy hitter, because that way, you don’t have to always be that character who is only good for damage. You can heal, and create crazy items that will be great for the gameplay. And even if you are a half orc, the fact that you are now a tinkerer will add great role-play potential for your campaign.

i would do anything without magic maybe kensei monk would work
Something I've always wanted to play (but it's MAD as eff) if I ever play an over the top, more than level 20 character is a Monk-Barbarian-Paladin. Six levels of Paladin for Aura of Protection, 14 levels of Monk for Diamond Soul and then Zealot Barbarian on top of that and you will be pretty much immortal. Especially if you play an elf or half-elf since then you can't be put to sleep with magic.
That said, depending on whether or not you will actually use gestalt characters, how levelling up works for you and what you rolled I would probably do a Zealot Barbarian/Paladin of Conquest (or Vengeance) to begin with. That way you can use armour until you get your stats up, get some free healing, fighting style (I suggest GWF and a great sword, then GWM) and not that far off you get Aura of Protection.
you could go blood hunter order of the lycan (wereboar for orc i always thought was cool). The blood hunter doesn't need an insalny high int, and the hybrid form boosts the bonuses from barbarian through the roof. this subclass is all about fuccen shit up, there's one feature you get the doesn't revolve around something you have being able to kick someone shit in, and its heightened senses. improved damage on melee hits, and if you ever loose that weapon cause of pesky shit, not too worry, its time to **** shit up with your magical predatory strikes. at 11th level you start regening when your below half. pluss blood curses, a fighting style, and an extra magical elemental dmg die. a brand that does bleed damage whenever a branded creature tries to dmg someone other than you and you know where they are at all times.

Undead Warlock - it's amazing, you can use it with Rage, and you don't need to buff Charisma too much.
Add a ton of in and out of combat utility... go pact of the blade. It sounds like you already have a 'legacy weapon,' so that's a perfect fit.
