The rules for Milestones are clearly documented in the Dungeon Master's Guide page 261 (emphasis mine):
MILESTONES
You can also award XP when characters complete significant milestones. When preparing your adventure, designate certain events or challenges as milestones, as with the following examples:
* Accomplishing one in a series of goals necessary to complete the adventure.
* Discovering a hidden lo cat ion or piece of information relevant to the adventure.
* Reaching an important destination.
When awarding XP, treat a major milestone as a hard encounter and a minor milestone as an easy encounter.
However, when I select this option when creating a new character, it does not work as per the rules (assigning XP by milestone).
Instead it works more like Level Advancement without XP: Story-Based Advancement:
LEVEL ADVANCEMENT WITHOUT XP
You can do away with experience points entirely and control the rat e of character advancement. Advance characters bas ed on how many sessions they play, or when they accomplish significant story goals in the campaign. In either case, you tell the players when their characters gain a level.
This method of level advancement can be particularly helpful if your campaign doesn't include much combat, or includes so much combat that tracking XP becomes tiresome.
…
Story-Based Advancement
When you let the story of the campaign drive advancement, you award levels when adventurers accomplish significant goals in the campaign.
Please either fix to work as per the Milestone rules, or if it simply has the wrong label, then change the label to the correct rule ("Story-Based Advancement").
(Edited to add a quote of the relevant rule text, rather than the phone cam picture; hopefully this is okay, as the context of the specific rules is useful to understand the bug / issue)
@StormKnight hopefully it is okay to at least quote the rule text, rather than a phone cam picture of the page, as the discussion doesn't make much sense without the context of the specific rule about how Milestones work in the game (as per the DMG rules).

Thanks.
P.S. I'm not sure if the above rules are available on D&D Beyond, as I don't have purchased access to https://www.dndbeyond.com/compendium/rules/dmg/running-the-game#ExperiencePoints (however, I presume they are the same).

Yeah, it seems to be a misnomer. Everybody understands it as "level advancement without XP."
