Adding a tooltip or updating the current version of the incapacitated condition to include the fact that it automatically makes you lose concentration, would assist when referencing the conditions in the players handbook.

Being incapacitated or killed. You lose concentration on a spell if you are incapacitated or if you die.

The tooltip for the incapacitated condition is taken from how the rules describe the condition. D&D Beyond can't change how the rules are expressed
