I'm running the Tyranny of Dragons campaigns with few of my own add ons, a few of the add ons were giving npcs and villains more character. My players have gotten attached to villain Cyanwrath and they are getting ready to fight him. I am thinking of having Cyanwrath return later in the campaign as a fiend any thoughts?
Like... he dies, his soul goes to Hell, meets Tiamat, and she turns him into a Fiend? I think that's a cool idea.
Something I did in my campaign, partly because I know my party are a bunch of softies who would have tried to rescue or raise the Black Dragon Eggs early on, was to show that the black dragons were corrupted by Tiamat and mutated, and that mutation took the form of extra pseudo-heads starting to grow from the base of the hatchlings' necks... it had a bit of a grotesque body horror element, but also made clear that killing them was probably the kinder thing to do.
Anyway, with that as a jumping off point... I think it would be cool if Cyanwrath comes back and suddenly he's got another head. He's been imbued with more of Tiamat's power, which manifests with a whole extra head of a different color appearing. Other than that it's just a matter of giving him a few more fiendish features... it depends on how late into the campaign you want him to reappear, but I'd say just find a Devil of the appropriate CR and use that as the base for Cyanwrath when he returns, and give it a few of his personal features as well.
Seems doable, since Tiamat still holds some kind of power in Avernus. Maybe give him the stats of a Blue Abishai, or an Erinyes if you don’t own the blue abishai statblock or want him to be a lower CR.
Okay I think I got it when Cyanwrath comes back I will use an adapted Erinyes stat block for the base stats, and give his armour a organic feel and static surrounding him. I will give a hint that Cyanwrath's isn't quite right by having him electrically disintegrate when he dies.
I'm going to incorporate Cyanwrath into the Cult strikes back part of Rise of Tiamat, which is a group of encounters that can happen any time throughout the campaign, with Cyanwrath taking part in the final attack. There will be hints throughout the adventure that Cyanwrath is back, with the earliest its hinted at being by Rezmir at the end of Hoard of The Dragon queen.

But I could always use more advice.
I think the other question is... where do you plan for him to make his return?
