Kinda weird how giants dont have stats on eating people. It sounds weird but it nakes sense, especially with the hill giant because their whole thing is eating. So would it be breaking the rules ti give giants the swallow attack opition?
I don't think a Hill Giant's mouth/throat are large enough to completely swallow a medium sized humanoid. A Remorhaz (same size as Hill Giant) or Giant Frog (smaller than a Hill Giant) have the ability because their anatomy is designed to do so.
I'm much larger than my son's python... but no way in hell could I swallow whole the rodents this thing does!
Your game though... and when I think about it, I believe I would allow a giant to swallow a halfling or gnome - way too much fun can be had at the table from those types of shenanigans!
Kind of bouncing off what Urandom said, giants share the same anatomy (assumed at least based on their design) as humans and so one thing we are known for is having to chew our food.
you could probably instead give them a chew attack that offers players the chance to save throw out of the giant's mouth while each chew does xdx damage.

Giving giants a swallow attack probably wouldn't unbalance things... However, many swallow attacks include base attacks with grapple riders. Giving giants free grapples could put things out of whack. The attribute scores on giants make grappling extremely effective. In the base rules, giants don't get free grapples, and that creates a kind of balance in that they need to give up attacks to grapple. If giants get free grapples, they gain a strong option and can use it while doing their full damage potential.
