Certain attacks from certain creatures will give you the poisoned condition - for example, the Ghast's stench till the start of your next turn, the Ettercap's bite for 1 minute. Poisonous beasts (Giant Scorpion, Poisonous Snake) often only give you the poisoned condition if they reduce you to 0 HP with their poisoned attack.
Poisons can also be ingested in specific role-play scenarios or placed on certain weapons. Again, there will be a description if it occurs.
I should also say you can craft poisons (or perhaps buy) that may (or may not - some give paralyzed or other conditions) give the poisoned condition, often for up to 24 hours like Assassin's Blood - you can find them in the DMG. They're expensive and difficult, though.
Periodically I'll run across a magic weapon that adds +xd6 poison damage. Does this automatically also give the target the Poisoned Condition?
