The group I am running Descent Into Avernus for consists of a Tempest Ceric, Celestial Warlock, and a Draconic Bloodline Sorcerer. All are seasoned players that have been playing with me for years. They had decided to leave LuLu, who has a good portion of her memories and abilities back, at Fort Kncuklebone for safe keeping since she had recently been kidnapped by a couple of nygoloths.
They are currently level 9 and got to Kostchtchie’s Maw. Sorcerer goes "It's just a chain devil and 4 hell hounds. This is gonna be easy." End of the encounter sees the cleric dead, the warlock unconscious but stable with 2 failed death saves, and the sorcerer at 17 of 94 HP with no spell slots or sorcery points left.
Hm. Ignoring the amazing sorcerer hit points, have they run into those creatures before? Because a 12,000 xp encounter against 3 level 9s isn't something I'd consider 'easy'.
Reminds me of my friend who back during the AD&D days when you had to roll your stats, rolled his by himself in his house and apparently got "so lucky" with an 18/00 Strength for his fighter.

Reminds me of how my Level 8 party lost a character to some kobolds last session because they thought they'd be easy enough that they didn't have to short rest. Turned out the kobolds were very tactical! Good on you for not pulling punches.
Yes both chained devils and hellhounds are earlier in the campaign but previous encounters only had 2 Hell Hounds. This encounter though the Chain Devil has more animated chains and the a whole pack of Hell Hounds.

Draconic is 1d6 +1 + Con bonus. With a 20 Con, that is reasonable. 6+ (8*3.5) + 9*5 + 9 = average 88, Max 108
That just shifts it over to the sort of stat rolling that results in a 20 Con at level 9...

He has an Amulet of Health so 19 CON.
