Now this isn't a cheap item to make as it uses many instances of this spell, which costs 10gp each, of jade dust, but if you make a circlet that has a ring of items in it, each item can be made to hum quietly when (insert here, I like person with evil intentions toward me or my party, or a specific person you need to track) when the person is within the range (30ft) in that direction.
So, when worn, if someone who fits the trigger comes within 30ft, I hear a hum in their direction and can turn my head to home in on the trigger. can also use more items to make different pitched hums depending on the range, so you know the direction and distance. and a difference pitched noise to determine altitude if you want.
This circlet, mechanically, lets you know the precise position of something of your choice within 30ft
and with more wording and items, you can narrow down the weapon, armor, race, gender, and whatever you think of.
remember it does cost 10gp per item, can if you go too far a dm can say you have to roll perception to distinguish the noises, though that can be mitigated with actual words, like "5ft" "sword" and other words that would work, and a dm can make you roll on the madness table if you have too many noises.
A lot of this is a stretch. The mouth only responds to “visible or audible” conditions. I don’t think it can distinguish between someone with hostile intentions or not, as that is not necessarily visible or audible — an ally can have a weapon out, something sneaking up on you might not be visible or audible. Also, it just lets you know if the trigger is within 30’, nothing about direction or precise position. So you could, I guess, step to the side to try and get in and out of the radius, but that would be pretty inefficient.

"Person with evil intentions" is not a visual or audible condition. "Person I need to track" might be visible, but the magic mouth doesn't have a perception skill so it's not going to be better at spotting people than the owner.
Generally speaking I wouldn't let you apply particularly abstract conditions/characteristics to the spell. I'd aim closer to the high end of what a computer program can recognize at the outisde: X symbol, Y gesture, etc. Basically if you could provide a picture or video of what it's looking for, it's good.

If they come behind you, the item behind you will sound first before the other three, telling you that they're behind you. You could then, theoretically, fine tune it with additional such magic items on the frame to get more precise directions.
I mean, I don't think it would work for various other reasons, but the direction thing is part of the OP's design.

That way I can hum it out loud at the table, annoying everyone, and it's totally in-character.
