This is correct. You get the Extra Attack feature once. If you multiclass and would get the feature a second time, you do not get a third attack-- You still only get the two attacks.
But what about abilities besides extra attack, like flurry of blows, dread ambusher for ranger, etc?
Those do stack.
The general rule is one calculation. (This is often discussed in regards to AC calculations.)
Extra Attack: attacks per attack action = 2 Thirsting Blade: attacks with pact weapon per attack action = 2 Flurry of Blows: after taking the attack action, unarmed attacks per Flurry of Blows bonus action = 2 Dread Ambusher: first turn in combat: +1 to attacks per attack action
Thirsting Blade and Extra Attack wouldn't stack since they both say 2 instead of 1, as opposed to a bonus ("make one additional weapon attack as part of that action").
e.g. a level 5 gloom stalker ranger/level 2 monk can make up to 5 attacks on his or her first turn in a combat. 3 { attack action [ 2 (Extra Attack) + 1 (Dread Ambusher) ] } + 2 { bonus action [ 2 unarmed (Flurry of Blows) ] } = 5

Fighters get Action Surge, allowing them to occasionally take a second standard action on their turn.
A level 5 fighter has 2 attacks per attack action (Extra Attack). Via Action Surge, two attack actions can be taken on the same turn, plus a possible bonus action attack(s) if utilizing two weapon fighting/Flurry of Blows.
As per the Paladin getting more than two attacks, two weapon fighting can add a third via a bonus action. Also the Haste spell gives a second action which can be used for another attack (only one attack per the spell description—no Extra Attack).
When not using a two-handed weapon for attacks you can hold it with one leaving the other hand free. So yes the monk could make unarmed attacks with their other hand or kicks or headbutts or whatever.
But if I use the two handed monk weapon to attack in the same turn as an attack action, I couldn't use my bonus monk action to make an unarmed strike?
Incorrect. Holding the weapon with two hands is only required for each individual attack - not the whole action. As soon as each swing is done you can let go with one hand. And even if you were required to hold it with both hands for whatever reason - you can still kick or headbutt so it's fine - you would be free to bonus action unarmed attack or Flurry of Blows as you wish.

2 levels fighter for action surge (+1 action per short rest), 2 levels monk (flurry of blows) but cant smite, 3 levels ranger for dread ambush (+1 attack per action on first turn of combat), 11 levels fighter (3 attacks per action, full stop).

Monk is pretty good. In terms of DPS, it will be out matched by fighters at high levels. In terms of burst/Nova damage, paladins reign supreme. But monk has a good pool of features and can really amplify the other melee classes

The rules say number of attacks don't combine, and abilities that grant extra attacks like the warlock's thirsting blade doesn't give extra attacks.

As a rule of thumb extra attacks granted by features that are not the "Extra Attack" feature do stack with multiclass.
The 2 you have stated definitely work anyways.

A level 5 fighter could also make 5. With 2 monk levels it would be 6. There are several limited use ways to get a burst of attacks.

Sure, fighter 2 / paladin 5 can make 5 attacks on their turn once / short rest. Once per day all 5 could be smites.
