So I have a wondrous item outfit that has a opportunity attack built into it. (item listed in spoiler tag)
Adding a modifier for the +1 AC is easy enough but for the life of me I cant figure out how to set up the OA so that it shows up under the reaction section of the character sheet when the item is attuned. And to be honest I cant figure out the bonus action either but that really isn't as important to me right now. About the only solution I can find is to actually equip the item and attune to it, then go to the character sheet and make a custom action with the benefits listed but this a roundabout method.
This ancient officers uniform is heavily accented with bronze and silver and exudes an aura of authority.
This outfit has 5 charges. While wearing it, you can use an action to expend 1 or more of its charges to cast the Command spell (save DC 15) from it. For 1 charge, you cast the 1st-level version of the spell. You can increase the spell slot level by one for each additional charge you expend.
The outfit regains 1D4+1 expended charges daily at dawn. If you expend the last charge, roll a d20. On a 1, the outfit retains its +1 ac bonus but loses all other properties. On a 20, the outfit regains 5 charges charges.
In addition while attuned to this outfit you gain the following benefits.
While wearing no armor or a shield you gain + 1 to AC
While you are wielding a one handed range weapon, or a sword with the finesse property, other creatures within 5ft of you. Provoke an opportunity attack from you when they move more than 5ft away from you even if they take the disengage action.
As a bonus action you can cause the uniform to billow dramatically outward behind you.
Well at least I can stop looking now for it. That's sort of disappointing but not the end of the world. Hopefully they get added in as options in a update.

EDIT: As a workaround I created the homebrew item then added it the items reaction property, not to the item but as a custom action for the player attuned to it. The reaction property for the item now appears in that character's actions and reactions tabs :) - a bit of a kludge but it works well enough for my purposes.
Two years on, is there any way now to add reactions to magic items? Would be really good to be able to set a reaction attack, as per the existing magic item "Sword of Answering".
It feels like they really phone it in on the Homebrew features. Every time I go to use the system it just results in me getting frustrated because there is no straight forward way to achieve what I need for the item. Since this thread is two and a half years old and no progress has been made I guess I'll be looking for cheap workarounds yet again.
The lack of this and so many other features (and bugs) on item creation is quite frustrating. To me it seems they are focusing too much time on things that look good for marketing, but not actual functionality anymore.
I'm making a kuo-toa sticky shield as an item (magic item, since we still can't make non-magical items), and want to add the use of it as a Reaction. Would be great to have this ability part of the Beyond item creation system.

This still isn't in the homebrew system, and it isn't even part of the published content yet. Call of the Netherdeep added the Ruidium Shield which grants an ability that uses a reaction, and it doesn't even add anything to a character's Reactions section.

Any type of action or reaction can't currently be set in magic items.
I don't know when they are going to be a feature of the system of magic items.

As a hacky solution, you can create a homebrew spell that casts as a reaction, and then add that to the item. Seems to work okay-ish.
