That’s how I have typically seen it played. A hidden rogue can pop out from behind cover for a ranged attack at advantage but if they try to move toward the NPC during combat without cover then the NPC will notice them.

Usually, during a combat, a PC trying to hide rolls a Stealth check to compare against the NPC 's Passive Perception.
A Perception roll would require the Search action by the NPC.
Almost all right but the definition of Passive Perception.
Passive Perception = 10 + Perception modifier
From a statistics standpoint, they are the same thing, as 10 is the rounded down average result of a d20.
