Every time I roll a constitution saving throw, I get a prompt asking if it's for concentration. Without fail, I have to interact and close it. Is this a bug or is there a setting that I'm just not seeing to turn that off?
Does the character have anything like the War Caster feat that makes CON saves different (eg. with advantage) when they’re for maintaining concentration?
If you choose “yes” at the prompt, do you see anything different about the roll (advantage, bonuses)?
If it’s coming from something like a feat, you may be able to get rid of it by making/using a homebrew copy of the feat.
