On my level 15 pal/4 warlock, I noticed today for the first time an item in my actions that says "Selfless Boon (Special)". I have no idea where that came from, not from any of my feats, species, abilities, or anything. And when I click on it, it gives an error message. I have no homebrew items like that, and the character is not in a campaign with someone else that has home brew items.

Are you entirely sure you don't have that feat on your character? It may have been added via the "Manage Feats" button on the character sheet rather than via the character builder.
