You can try something like what I did here. It's a magic item (I made it legendary so it appears at the top of their inventory, but it also weighs 0 lbs and costs 0 GP and does nothing). The version I made has 4 charges to represent up to four days of long-term madness. The RAW for long-term madness are 1d10 x 10 hours (i.e., up to 100 hours, just over four days, and as little as 10 hours). Personally, I find reliance on hours to be annoying for long-term madness, so I made it based on days (and therefore long rests). When a character develops long-term madness, I instead roll a 1d4. If I roll a 3 or lower, I tell them to automatically check off 1, 2, or 3 charges on the item. From there on, each time they long rest they may check off another charge. Once they've checked off all the charges, they may delete the item from their inventory. It's on my players to remember to check off charges, and so long as they still have un-checked charges on the item they will continue to suffer effects of long-term madness.
It's not a great solution but it works well enough for me!
