I understand that this class is critical roll content, but why doesn't D&D beyond track the hybrid form melee damage on the character sheet.
I understand that you can make custom attack options with some control but shouldn't an official material of Beyond have it's hybrid attack already represented on the Attack actions tab.
What have you guys been using as a workaround? Some sort of reminder?
A work around I see people use often (especially for rage and mage armor) is make a magic item with modifiers for the temp bonuses and equip and unequip the item accordingly.

That is perfect. I'll be doing that.

Is this something that has been addressed?
The character sheets on DDB currently can't track temporary conditions like hybrid forms. It is a planned feature. And once it can, I bet it will.
