Damn that is pretty good! And extra +1 to Spell Attack Modifier and Spell Save DC is the same as moving up a point in Ability Modifier to Wisdom, which would happen at Level 8 (assuming your player is already Level 5), or your Proficiency Bonus going up by 1, which will happen at Level 9 for a Druid. So yeah I think it's pretty powerful, they're getting a big bonus a few levels early, but could be balanced, depending on your game. The fact that it requires attunement is good, because that limits the other magic items your player could use.
The one thing I'm curious about is this line `While the creature is summoned but concealed within the amulet`. What does that mean? I think it might be more balanced to require that the familiar be out and visible, because then if it dies, the player looses it's bonus until they have an opportunity to re-summon the familiar.
That said, I really dig this. I think I'm going to incorporate a version of it in my home game! Nicely done!
Seems OK, balance-wise. It's maybe about as good as a Rod of the Pact Keeper (the spell slot recovery being replaced by the general usefulness of a familiar), which I think is fine for a level 5 Warlock.
(Just an unrelated suggestion... most items which require a specific class and require attunement are phrased: "Requires attunement by a Druid", rather than the "Extra requirement" you used in the description. See the Rod of the Pact Keeper for an example.)
I almost add the +1 to wisdom, but that also includes lots of extra skill checks, and that would be too overpowered.
Maybe it is not explained good enough. The idea is that you only get the bonus if th blight is summoned, but stored it in another plane (just how the spell works) , so you can’t have the bonus while having the creature outside. If you want to use the familiar you loose the bonus. If the creature dies, you also loose the bonus until you cast find familiar again (also spending gold in materials)
The player can choose either to have the bonus or the creature with a bonus action
I agree with hermes: the bonus should be applied when the familiar is summoned, rather than when it's dismissed. I understand your intention (to create balance, to force the player to make a choice), but in practice, it will probably work the other way around: the character will usually have the familiar hidden while in combat, to get the bonus, and then summon it out of combat, for utility. If they're tied, rather than exclusive, the character needs to protect the familiar while in combat, to preserve the bonus. Out of combat, the bonus will rarely be an issue. Or, another option is to have the familiar have an extra action it can use on its turn: to grant the bonus to the character. That way, the choice still exists (use the special action, or do a Help, or scout, or.. ?), but the tension of having to protect it in combat is also still there.
Also, to protect against loophole exploiters, might want to include some clause about not being able to re-cast the spell to change the familiar's form (Find Familiar normally allows the caster to re-cast it to change the familiar from, say, an owl to a, for example, cat), unless it's intended that they be able to do that.
But that's really not a balance/power issue, which is what you asked for. :)

Option 1 - If you have to know and cast Find Familiar on your own - which is not a druid spell...
When you cast the Find Familiar spell you may use this amulet as an additional component, which is not consumed by the spell (the standard components are still required and consumed). If you do, your familiar may take the form of a Twig Blight.
Or, if the amulet gives you the ability to cast it:
Once per long rest, You may use the amulet to cast the Find Familiar spell. You do not need to know the spell or use a spell slot but you must still provide the required material components, which are consumed as per the spell. When you do, your summoned familiar takes the form of a Twig Blight.
Then, either way:
As a bonus action, if within 5 feet of your Twig Blight familiar, you may merge the familiar with the amulet, the Twig Blight is sucked into the amulet, disappearing. When merged with the Twig Blight, the amulet grants you +1 to your spell attack modifier and Spell Save DC.
As a bonus action, if your Twig Blight familiar is merged with this amulet, you may cause it to un-merge and appear in an unoccupied space within <some range you decide>.
Your Twig Blight familiar, in all other ways, acts as a standard familiar per the spell.
