you read the title, Im making a domain of dread were nothing ever dies permanently, but because this is a domain of dread, the resurrection can go wrong. currently I have this table

You return with one minor physical difference (eyes being a different color etc.)

You return with a random dark gift (reroll is you’ve already rolled this)

Your scent changes to a wet, muddy, mushroomy smell

You return with your organs rearranged

Animals now shun you

You gain or lose a finger or toe that doesn't obey normal geometry
13-14
Part of your body is composed of a strange material
15-16
You return as a random lineage (see chapter 1)
17-18
You return with on of your ability score permanently reduced by 1

The Staring cats of ulun-dar now haunt you. These Cats are non-hostile, possess a variable amount of eyes, & regale you with tales of how you died in other realities. Only you can perceive the cats. The cats can be dispelled with Dispel magic of a suitable level or similar magic

Can you define resurrection? Do you mean only that spell? What about raise dead or revivify or true res? Or is this just for if people basically re-spawn by the rules of your domain? I’m not sure that having organs re-arranged actually matters. In the absence of called shot attacks and surgery, I doubt anyone would notice. Losing an ability score point seems a bit harsh, or potentially meaningless. But if you were, say a cleric with a 15 str, and now you can’t wear your plate mail anymore, it would be really rough. But if that same cleric sees their cha drop from 10 to 9, it may not even matter. You want something that would impact all characters equally, where this idea seems pretty swingy. Maybe you lose it for 1d4 days or something.

sure. when something dies in the domain, their body sinks into the spongy ground and is integrated into the walls of an overgrowing subterranean maze. then, a couple of days later, they wander out of the swamp, their new body formed by the mists. I like the 1d4 days idea. and something I didn't mention is that everytime you die here you roll one more d20 on the table and take the highest number, so the more often you die, the worse you get

Keeping in line with general DND behavior and mechanics, you should roll multiple times and take the lower of the two instead of the higher. And then just flip flop your table.
I don't think there's anywhere in the game where a low roll is good, so to keep things consistent I recommend that.
You also, to my knowledge, don't have any effect on rolling a 19.
