So, if there are any other helpful DMs out there, I'd really like a hand at compiling examples of what it could be that I'm thinking of, here. Spells, currently-existing Magic Items, Feats, and...if we really have to...Class Features. I'd like to create Homebrew Magic Items that detail these abilities, and if it just has to be through text descriptions, so be it. But if those text descriptions already exist somewhere, well...work smarter, not harder, am I right?
Hi. I think that I can help with this. I just saw this, and I'm looking at something that might work. Cape of the Mountebank (which is rare) uses Dimension Door, so in theory you could go to another person's spot. What if you made a magic item that let you and an ally within a certain range (and within sight) swap using Dimension Door? It would essentially create a portal at both ends that you would step through and it would vanish and you would swap places (or squares, or hexes, whatever you're using). You said you're looking for a magic item, right? I think that a wand could work, since it's the most practical. It would easy to carry and hide, and then you could teleport if need be using it. You could call it the Wand of Switching or something like that. It doesn't have to be Dimension Door, though. Any teleport spell (Misty Step, etc.) could help. Hope this is what you're looking for, and good luck!

I'm aware that it's easy enough to create in my own words, but I'm hoping to recreate it from already-existing effects, especially if they are Magic Items, Spells, or Feats that I can copy as close to whole-cloth as possible, though I appreciate the suggestion.

Dimension Door (And Cape of the Mountebank) isn't a bad idea, but I'm looking for something closer to specifically what I was asking for, rather than something that can be cut down to fit the mold. I know there is more than one thing specifically out there that I have in mind, I just can't remember what it is, and it's driving me nuts. Still, I appreciate you taking the time to chime in with the advice on the matter.

Hm. This might be it. I could have sworn that I was thinking of something more along the lines of a reaction, but there's nothing saying this couldn't be turned into one. Still, I was really hoping it wouldn't be a Class Feature - these are a real ***** to convert into anything else. That said, this is the closest to what I was trying to remember. I'm going to keep trying to figure it out, but still, thank you. I don't know if it's exactly what I was looking for, but you're pointing me at least towards the right direction, and I really appreciate that.
I believe that the only thing that lets you swap spaces with a willing creature is Benign Transposition. There are spells that allow you to teleport, and there are spells that make other creatures teleport (vortex warp), but beyond Homebrew there isn't much swapping of spaces going on (although Bait and Switch does some swapping, though it's a Battle Master maneuver and it only swaps adjacent creatures)
