So I looked at the places you can find them. You can find the lost in the desert, so does that mean they can manifest in the martial plane or can attach themselves to people on it as long as they are sharing the same feelings?
Yeah but you know that section where it tells you where to find those creatures? it said that some of them an be found in the desert. Yes the shadowfell spawn them but maybe they can latch onto someone lost in the desert?
In theory, anywhere. They originate in the Shadowfell, but there are crossings that lead from the fell to the Prime Material (A lot of them in Neverwinter, actually). These crossings can appear anywhere, but especially places filled with despair, such as a desert where many have died.
While the lore lists them as being created in the Shadowfell, they are tied to the negative emotions that created them. They could be drawn to areas strong with that emotion, so anywhere you as the DM would like them to show up. Places of war, suffering, or death.

Aren't they formed from a combination of negative emotion and shadowfell energy? I don't have Mornenkainen's but that's my two cents.
