I'm running a Flameskull this week and the statblock states the flameskull has multiattack. So when it casts its Fire Ray it can cast it twice. That part is pretty clear.
But the Flameskull also has spells. Can the spells also be part of the multiattack feature? For instance, can I cast Blur and then Fire Ray because of the multiattack?
Nope, for one thing Casting a spell and Multiattack are two distinct actions (the latter being exclusive to NPCs), and a monster's stat block says exactly what it can do when using the Multiattack action (Fire Ray twice for Flameskulls).
A monster can only multi-attack with the things it says the monster can multi-attack with. If it has a sword and a dagger, even, and it says "Multi-attack, it can attack twice with its sword", then it can't attack once with sword and once with dagger. Only what it says in the statblock.
