The domain of Klorr from Van Richten's guide really interests me. I love that kind of surreal horror and feeling of impending doom, but I really struggle to figure out anything for players to actually do there. Maybe I'm just uncreative, but it seems like a world where every possible area you can visit is constantly one hour away from doom and you can't even find the Darklord would be kinda pointless to adventure in.
What happens when an island is knocked out of alignment? When the perfect clockwork of the domain is disrupted? What fate awaits the residents of the islands, and how will the dark clockmaker react to the disruption of his doomsday clock?

It depends on what kind of campaign you're running, but the ticking clock scenario Klorr embodies would be a good place for a heist... the detritus of every other Dark Domain seems to wash up there, so the party has to go in, find the thing they lost/failed to get in a prior mission, and get out before the island its on gets consumed. All the while you could toss in references to other Domains they've been through and people they met (killed?)
Beyond that, the place has a real Borges vibe to me, so I'd probably lean into that with lots of mirrors and labyrinths and such

You could have the characters trying to escape an island before it's destroyed.
I have a weird sense of humor.
I also make maps.(That's a link)
