Spears are considered "Versatile" weapons, which means they'll do slightly more damage if wielded with two hands. Choose the appropriate dice value according to how the weapon is being used.
If the enemy wields a shield or holds an offhand weapon in their other hand (Dagger, Hand Crossbow, Focus etc) then you use the lower damage roll as their using the spear with one hand. If they unequip their shield/ offhand weapon they can then use the higher damage roll when attacking. If they only wield a spear they use the higher damage roll provided their not holding onto anything else with their other hand.
On the stats of the Diamond Knight it displays:
Diamond Spear. Melee Weapon Attack: +3 to hit, reach 5 ft., one target. Hit: 7/9 (1d8/1d10 + 3) Slashing damage.
What would it mean to have two different number of Hit (1d8/1d10 + 3)? I don't see anything of double attack on the stats.

Separately, how they got 1d8+3 = 7, 1d10+3 = 9, is a mystery (1d10+3 should be 8).
