It's hard to form an opinion without context what level are the players - what is their power level are they decked out in artifacts - are you running a post l20 campaign?
An Ancient Red Dragon (CR24) fighting along side a group in anything other than a super high power late end game group or Epic group is game breaking by definition.
If you plan to nerf the dragon then just go lower on the dragon scale like a young dragon or something.
Assuming you make the Fire Giants immune to Fire (per the stat block) and make them immune to the fear (Or what's the point of the encounter if they are all feared round 1)and put 4 or 5 of them on the map to handle the dragon - then I would say balance the fight as you normally would for the group with the additional 4-5 creatures on the map to handle the Dragon but that would be a lot of creatures to control. Also if there is a healer in the PC group will he be healing the dragon - lots of variables I would be fluid in the Fire Giant HP blocks start at 200 then see how it goes after a few rounds.

Outside of lair. They are inside fire giant lair. So flying above say 40-50 feet will not be possible. Assume large 300’ x 300’ x40 combat area.
