So first off not a new DM but newer to DMing in 5e. Ive been looking around online trying to find the perfect DM sheets for my screen. Important references like conditions, combat rules, etc. Not interested in random encounter sheets as I'm pretty prepared for those scenarios.Curious to see what you all use for your DM screens! I like to keep a few of my player character's traits like resistances to damage and condition immunities close at hand, other things I like to have is things like fall damage rules, simple traps etc etc
The essentials kit has exactly what you want, but there's a small drawback. Being from the essentials kit its made of basically paper, mine at least doesn't fold well, and it's a little short for some people. But it has everything you asked for.
Not going to lie, I haven't used it because I play online though, but it has what you wanted and much more.
I purchased the core rulebook set (PHB, MM, DM Guide and DM Screen). I use that screen pretty exclusively. There are some things I like to have that it doesn't and some of what it lists I never need. However, I use that area to clip a running list of the player's HP and AC points. I keep initiative down at the table, though I have been toying with the idea of clipping the orders to the top of the screen so everyone has an easier time keeping track of the combat orders. However, a quick google search will turn up hundreds of ideas on layouts if you want your own custom sheets to put on the back of yours. Someone will likely post theres, but I just keep it simple with what I have.

Here, have some homebrew classes! Subclasses to? Why not races. Feats, feats as well. I have a lot of magic items. Lastly I got monsters, fun, fun times.
