Being hit with a physical attack by a "nasty" creature (ie; wereboars, rats, spiders,wererats and such) has a chance based on the PC's Constitution mod to infect them with a nasty disease. Diseases, if not cured through potions or magic, can lead to death in several circumstances.
Unabridged travel-The PC's actually have to navigate throughout the whole of the world map to get to other places. If they're lucky, they may find hidden tunnels that go underground to get them there faster. If they're unlucky? Those tunnels won't be empty...
Restless sleep-anytime the PC's are taking any rest anywhere but in a town, village or city; they have to roll random encounters throughout the rest period.
Isolation-Being alone without a group or adventure buddy for too long will cause insanity.
Fatigued-You're wearing armor that's really just too heavy for you, I'm sorry. +1 level of exhaustion until heavy armor is removed and a short rest is taken
Maiming-before you die, when your hp falls to 5 or less points, you'll be maimed. You can still move for half of your walking speed and suffer a -3 to all your stats. The only way to recover from being maimed is by magic or potions. Bed (long) rest doesn't help if you're bleeding to death. Ya know?

Humiliated-if you suffered a failed save or DC the turn prior, you're too embarrassed to try again right now. You'll have to wait one full turn before attempting that save again.
Waterlogged-If PC's are wearing cloth pants or shoes and have to wade through deep water or mud, their clothes become waterlogged and you suffer -2 to dex and -1 to initiative until your clothes dry.
Shocked-If a creature or monster or whatever that is one character size larger than you starts to dash in your direction, you must move out of its way or be trampled for 1D6 bludgeoning and you become shocked. While shocked, all you can do your turn is roll a DC16 Constitution save to snap out of it. Only other way is for another party member to strike you themselves.
This is going to take a while to do all by my onesome.
Wearing metal armor or metal studded clothes outside during a thunderstorm can get you struck by lightning. Each turn out in the storm roll a DC 15 con check. On a success you don't get shocked. If you fail the roll a searing hot bolt of lightning strikes your armor or you. take 2D8 electrical damage. (If that's not a type, then radiant I guess IDK)
If holding a metal weapon and struck by lightning whether magical or natural, you convulsively clamp your fingers down on the grip and suffer 1D8 of afore mentioned correct type of damage for 2 turns.
If fighting a Mature or better dragon and you're in melee range of their head. Dragon can roll for a snap attack and on a 15 or higher, the dragon gobbles you up whole; INSTANT DEATH!
I'm only worried about it, because the last person that helped me when I was trying to write them in a notebook, conviently left my house with the notebook with my sole copy of those notes and rules and I never saw them again.
If not for that theft, I would already have most of the rules down on here. I had a 70 page college ruled notebook with nearly every page full of rules and stat boosts for enemies both humanoid and otherwise. That broke my mind when they stole my framework.
I'm assuming that since you aren't saying anything back, that it's a mod, and I'm sure you guys wouldn't pilfer somebody else's hard work. You're doing a good job this evening by the way.
Crime doesn't pay!-If you're in a town and get caught trying to steal something or pick a pocket, you get your hand cut off. Really cliche, but you can't argue with the results.
Exhausted-If you dash more than twice in a single combat you take -5 to initiative next combat unless you rest between now and then. Short or long, doesn't matter, sleep is important!
You know, if you're not terribly busy, I'd like to see what kind of hardcore rules a DDB mod could think up. I'm not trying to get you to neglect your job though, so just one if you please. If not, then I'll just pick up here tomorrow.
Come on! Step right up sir or madame, don't be shy. Think about the normal rules for D&D, then think about ways to twist or bend that rule to make the game much harder. Like the one about getting to close to the front end of a dragon means it can try to eat you whole for instant death. You can do it! I have faith in you. . .whoever you are lol

Permanent Death-when a PC dies, that's it. That player can either ask the DM for a prefab character or make their own all over again.

Demagogue Victim-If DM has given party members Inspiration, they loose it immediately and suffer -1 to initiative while in the demagogue's presence.

Counterfeit magical or legendary items look the part but do nothing at all out of the ordinary.
Knockoff magical and legendary items have a watered down or drastically hampered version of the original's enchantment.
I use both of these in the Trials of Dys.
So never trust a gnome wearing sunglasses and a big coat. He may try and sell you a watch...
