I am not sure. It was Matrick and Donavich that went in first with Patch going in then I guess Richmond when they tried to get him to open the door.
Would Patch have been within 10 ft. Of the door before she started moving to open it or was she farther away?
Yeah the trap door is ten feet from the door unless you are standing on the door.

If Patch is not stabilized she will need to roll a death save.
