1. The +1 is because you somehow have a "+1" magic Rapier (+1 weapons give "+1" to attack and damage)
2. "To hit" means the number you add to the d20 roll to see if you hit with a weapon attack. it comes from adding your Proficiency (+2 at level 1) Your Dex modifier (+3 Assumed) and, in this case, the +1 from your magic weapon you have (Total +6)
3. When you roll the Rapiers damage (d8) after hitting you add your Dex Modifier (assumed +3) to the damage (because Rapiers have the Finesse quality) and then add any modifiers from the magic weapon (+1 in this case) for a total of 1d8+4
The +1 means it's a magic rapier that gets +1 to attack rolls and damage rolls
The hit column indicates your to hit bonus, the number you add to your d20 when you make an attack. This will be your proficiency bonus plus your ability modifier plus any other bonuses (such as the +1 from the rapier being magic)
The +4 damage is from adding your ability modifier and any other modifiers (such as the magic +1). You don't add your proficiency bonus.

Thanks for the help. Point about the basic rules taken, but they are not the easiest to follow for a new comer and to translate to the character sheet. But I will try harder before posting this sort of question again.

I have created a Rouge using the Character builder (level 1) and in the Weapons section the stats for the Rapier read as follows:-
Rapier, +1 Hit Column +6 Damage column 1d8+4 Piercing.
What is the +1 in the first column?
What does the Hit Column mean?
Why is there +4 in the damage column.?
Sorry to ask basic questions but I need help!
