How are you getting a 2h duration out of your potion of speed and potion of storm giant strength combo? They have a 1 min and 1 hr duration, respectively. Durations are concurrent, not stacking; even if they stacked, it's still not two hours.
Had a character use several wishes to combine a longsword of sharpness with a Dragonslayer longsword and an intelligent giant slayer longsword - basically a ranger’s delight artifact.
Most insane item is an iron horn of Valhalla. Giving it to your players will completely break encounters by shear numbers. My poor, well thought out BBEG didn’t stand a chance. Even a silver horn of Valhalla is crazy

In my world (Dungeon Master) Adamantine has positive gravity effects due to high density, and Mithral has negative gravity effects due to because I said so. So if you were to combine them, you would have a piece of armour… that’s it.
Since this appears to be about homebrew content, none of the options listed come close to the insanity of a +1 Davy Crockett.

It was actually a recoilless rifle munition, not a missile. And required a crew of five. Did you really think I would make a joke about adding a tactical nuke to D&D without knowing what it was?
