Paladin with mage slayer is standing next an enemy caster. The enemy caster starts to cast a spell, 4th level spell. The paladin’s friendly wizards casts counter spell, the enemy caster counter spells the counter spell. Here’s my question, when the paladins mage slayer triggers on the enemy casters counter spell does the enemy have to make a concentration check to cast their action spell when hit by the paladin?
To cast? No. If they take damage and the spell requires Concentration, yes. If the spell fails, it doe the initial damage (or whatever effect), but anything that is preconditioned on continuing Concentration fails.

The only time a caster is concentrating on a non-concentration spell is if they're holding their action to cast it. Using your reaction while using your action does not require any concentration.
Since the mage slayer was triggered by the Counterspell, not the action spell, even if the action spell requires concentration the spell has not been fully cast yet so the caster isn't concentrating on it yet. The paladin should wait until the action spell is finished casting to use his/her reaction.
No Concentration in this case. It also may not work depending on what 4th level spell the enemy is casting because if it's a bonus action, he can't cast Counterspell.
