After eleven years of tabletop gaming and a year into this campaign, the Barbarian Warforged died, and was killed by none other than a puny skeleton.
It was awesome. Bittersweet, but awesome.
That character had been the lunchpin of the game, easily taking 40% of the kills. He'd rolled great, so he had +4 in Dex and Con and +3 in Str, and with Great Weapon Master, he could destroy pretty much everything. Despite being a light roleplayer, his player made the character into an integral part of the team; giving the halfling piggyback rides and being bros with the Bard/Warlock.
The other players want to find a way to revive him; I think it will morph into sort of another main storyline!
Oh, and his player is my roomie, so I had to meekly go out and say sorry afterwards xD

Oooh yep.
So, he is a Warforged Barbarian, with Great Weapon Master, Level 4. He's got great stats as I described, and he's easily been the main damage dealer of the team.
The enemies was an allip (the BBEG who's spirit asked for support from his evil demi-god and got turned into a crazed mass of black ethereal strips of cloth), four skeletons and two skeletons with more damage and armor that I homebrewed called Skeleton Warriors...
And he just rolled so badly. The skeleton hit him once, and he never hit the skeleton with more than about 7 damage. Meanwhile, the allip could easily fly around, dealing AoE damage and forcing other players to hit the Barbarian. I was lucky and got Howling Babble off a second time, which threw him down to 0 HP for the second time, and then he promptly rolled 1 on his death save. The skeleton took the chance on its turn and dived into Barb, plunging its rusted shortsword into his neck and ripping him apart (right in front of the Barbarian's best friend, an emotional, easy-going Bard/Warlock, who is certainly scarred for life now).
And that's how my player's AC 17, high health damage monster died to an CR 1/4 creature. Some fixy roleplaying with an artefact caused the allip to be desummoned with 3 HP left and the players were left with the sound of mad whispers and clattering bones reverberating in their minds...
Luckily, I knew he had another character ready to go that he kinda wanted to try out, and he's a bit of a gamer-gamer who likes when the rules are followed, so he enjoyed seeing my lethal side; I'm usually a Matt Mercer, narrative type player, so he liked seeing me go for the kill!
I killed my first character 3 weeks into my DMing journey, and i've killed 4 players in 2 years, only two of which were permanent, and one was to Strahd.
It's pretty incredible you made it 11 years!
Dying can tell awesome stories too, but sometimes a Skeleton just gets lucky, and while it can be a bummer, most times the player is ready and excited to reroll after about 30 minutes.
Your idea of the party going on a journey to bring him back is awesome, I had one of these recently where my player rolled a temporary character that helped them do it!
In my world, the Grey Elves are part of the Roman Empire (the other Elves are not and do not like humans very much, or at least Romans), and the party has spent the last 2 sessions in a Grey Elven port city. I described it as like being in a musical. The elves all sing while they are going about their daily work, and they will often sprinkle lyrics into their songs about what is going on, so the party walks by and they hear a line about the "little gnome with the squirrel on her shoulder" (the party's beastmaster ranger).
One of the players liked this so much she said, "If my character (human fighter) dies, I'm making up a singing grey elf!"
It can be hard to lose a character you like, but there are so many possible cool character ideas out there, that you can look on the bright side -- that character is done, now I get to make up a cool new one.

I don't think there is any RAW on killing players. Pretty sure it is DM Fiat.
