Am I missing something? Aren’t creatures that don’t have all its HP considered unconscious (or dead if a monster), thus anyone attacking them have advantage against them? What makes Blood Frenzy so special then? What am I missing?
Blood Frenzy. The blood hunter has advantage on melee attack rolls against any creature that doesn’t have all its hit points.
If a monster is not at full HP, then Blood Frenzy procs and gives you advantage.
I have 100 HP total. I take 1 damage from stubbing my toe. I now have 99 HP of my 100, and a Blood Frenzied Hunter now has advantage against me.

You're confusing having all of your hit points with having any of your hit points
A creature at 99/100 hit points doesn't have all of its hit points, so would qualify for the ability
