I'm taking over as co-DM for my group, and my regular DM/new player wanted to make a hexblade warlock with a somewhat general back story. I've made this based partly on his story, and partly on some of the weapons in the books and others posted here.
I know I will ignore the ruling that sentient weapons cannot be a pact blade (just for this game at least) - because I really like the idea of this character becoming an incarnation of fate, or even an avatar of the Raven Queen (even if for a short time), and I have made the process to elevate the weapon very tedious and somewhat difficult to balance that.
I was looking to have dormant charisma bonus at (+3), awakened at (+4) and exulted a (+5), and it seems I over looked it. Thanks for pointing that out.
My thought with the Death Ward and Dimension door for the Exulted level with the increasing charisma bonus, was geared toward the promise the general made to protect the wielders of the sword. Having an increasing option for the sword to take control and evacuate the wielder from a sticky/deadly situation seemed reasonable. My thought is to keep these abilities from the player, and only available when the sword is in control.
Should a sentient weapon have abilities separate from the players?

First question, what does this mean "and gains a Charisma Bonus."? How much of a bonus?
Also I think "Sword gains use of Death Ward and Dimension Door once per day" should be "Sword gains use of Death Ward or Dimension Door once per day"
