I am adding a bonus action concentration spell that gives the caster an extra body part that can do a melee attack (with the opportunity to do bludgeoning damage and grapple for the attack action, then poison damage for every round the target does not escape the grapple). The body part will also have its own AC and hit points. I honestly can't figure out how to add most of this into the spell, unless it's all things that just need to be remembered in the duration?

in additinal info you can add the attack type the spell allows you to make
you can add the grapple condition, poison damage and base damage pretty easily
the limb having hp and ac is something you will have to remember
This sounds not unlike Bigby's Hand, if you want an example spell to look at.
To represent things like a spell effect with HP and AC, I've taken to adding a creature under the character's Extras with the applicable stats, though that's done manually.

Using that spell as a base format helped! I'll definitely keep the added creature in mind too.
