The best way I can think of to do this is as a magical item with consumable charges. Is there a better way? Something that would scale with level maybe? Could be reset easily on levelling? Maybe even something to click on for the new dice or Beyond 20?

I think a Feat would be better. You can make a “general action” and set the number of uses through including the scale for level. It would take some work to set it up for every level, but it would do exactly what you want. You can put the 1d6 roll as an “other” actions (not Action or Bonus Action) to have it rollable on the new sheet. And the “Hero Points” feat could be added directly to the character sheet with no need for an ASI.
Edit: In fact, I just made that as a feat to test it and it works perfectly. I’d publish it for you, but that would break the rules. But I can talk you through the process.
I’m not sure I understand. When adding spells and I a feat they don’t get a “limited use” section the same way Actions do, there is just a field labeled “Number of Uses” (for a fixed number) and a dropdown for “Uses by Ability Modifier.” You can use either one.
If you meant under Actions (instead of “spells”) then first you have to create the action, designate a “reset type” and save the action. Then, when you re-edit the Action, all the way at the bottom a new “Limited Use” section will be available.

I see that this in now incorporated in the Actions section, so it seems they simply rearranged things a bit in the last 5 months. All good. My question is answered.
