Also a big thing: ALWAYS ALWAYS ALWAYS GIVE THINGS AN ACTION COST AND DURATION! Its the easiest thing to forget about or miss when first starting, but making something free actions-wise or accidentally infinite in duration is the fastest way to break it

I don't even know if that is possible without DM help. Lets see.
Level 20 barbarian can get a max con of 24, which is a +7 modifier. If you were allowed to take max hp every level, you would have 380 hp at the highest.
If you take the 2014 Mountain Dwarf as your race, you can add an additional 20 hp at 20th level.
Taking the Tough feat gives you another 40 hp at level 20.
If your Dm is allowing a mix of the 2014 and 2024 rules, and is allowing 3pp available on DDB, you could also take the Resolution of the Syndicate origin feat to gain another 20 hp, and then take the Boon of Fortitude epic feat for an additional 40 hp.
Thank gives you a grand total of 500 hp that can be achieved without some sort of magic item or other concession given by a DM.
