Do fighters using two-weapon fighting get the second (off-hand, bonus) attack with each attack?
Example, L5 fighter: With 1 extra attack action per turn do they get 3 attacks (1 attack w 1 bonus action for off-hand weapon PLUS 1 extra attack) or do they get 4 attacks (1 attack w 1 bonus action for EACH attack action)?
No. They take the Attack action, which may include one attack as normal, two attacks (level 5+), three attacks (level 11+), or even four attacks (at level 20). If any of the attacks they make during that Attack action are with the right type of weapon (light, melee) in one hand, while they’re also holding the right type of weapon in the other hand (light, melee), then separate from the Attack they can ALSO take the Two-Weapon Fighting bonus action, once on that turn. It doesn’t matter how many times you might qualify to make bonus action attacks, you nevertheless have but one bonus action to use per round on any of the various bonus actions you may be eligible for.
So your example of a level 5 fighter, they could attack twice during Attack, and once more as a bonus action using Two-Weapon Fighting.
Regardless of how many attack you can make when you take the Attack action , you only take 1 bonus action on your turn to make an attack using Two-Weapon Fighting.

I'm going to make this way harder than it needs to be.
