Anyone have rules on what happens if a character gains an increase to max HP wearing a magical CON buff item, then loses the item somehow? The CON modifier increase would allow them to gain more HP than they otherwise should be able to, so should this be accounted for when rolling for more HP? If so, if the item is later destroyed, do those HP retroactively get lost (similar to how HP are retroactively gained when a CON modifier is increased as part of an ASI)?
An example would be a player with a CON 15 (+2 modifier) wearing an amulet of health that brings their CON up to 19 (+4 modifier). This player would gain Hit Die + 4 HP each level instead of Hit Die + 2 HP without the amulet. If the amulet is destroyed after a few levels are gained, would their max HP be decreased by 2 x the number of levels gained?

Any time your Con modifier changes, your max hp changes immediately in response. Your current hp can't be greater than the new max, so if the new max is below your current, you recalculate current to the new max. Otherwise, your current remains constant when your modifier changes.

If your Constitution modifier changes, your hit point maximum changes as well, as though you had the new modifier from 1st level. For example, if you raise your Constitution score when you reach 4th level and your Constitution modifier increases from +1 to +2, you adjust your hit point maximum as though the modifier had always been +2. So you add 3 hit points for your first three levels, and then roll your hit points for 4th level using your new modifier. Or if you're 7th level and some effect lowers your Constitution score so as to reduce your Constitution modifier by 1, your hit point maximum is reduced by 7.
A creature's current hit points (usually just called hit points) can be any number from the creature's hit point maximum down to 0. This number changes frequently as a creature takes damage or receives healing. Whenever a creature takes damage, that damage is subtracted from its hit points.
