The Watchful Helm does not specifically say you cant be surprised like the Alert Feat. it says "remain aware of your surroundings even while you're asleep,"

and yes I know it add to passive perception which impacts surprise, just wondering if the "aware" part has any impact

It doesn’t interact with surprise directly at all. Your passive perception wouldn’t suffer any penalty from you being unconscious however.
