You gain an extra 1d12 Sangromancy Hit Point Dice per Vampire Transformation. You can use those dice on Sangromancy spells. You regain all spent Sangromancy Dice when you finish a Long Rest.
So this means you gain 3d12 additional hit point dice at 3rd level transformation? Or you receive 3 additional D12 hit point dice at 3rd level transformation.

What's the difference between 3d12 hit dice and 3 d12 hit dice?
I suppose the difference is, if you are a fifth level Fighter, you have 5 hit dice that are d10s, and if you read the feat as "you gain 3d12 additional hit dice" instead of "you gain 3 additional hit dice that are d12's," you would roll 3d12 and potentially gain 36 additional hit dice that are d10's, bringing your total hit dice to 41d10.

It looks like it would be 3 dice that are 1d12 each.

Exactly. I would certainly thing it means Three d12 hit dice.
Haha...phrasing.
